Effects of graduated compression stockings on cardiovascular and metabolic responses to exercise and exercise recovery in persons with spinal cord injury.
OBJECTIVE: To investigate whether reporting blood redistribution by means of graduated elastic stockings affects exercise and postexercise responses in people with spinal cord injury (SCI). DESIGN: Crossover trial. SETTING: Physical medicine and rehabilitation department in France. PARTICIPANTS: Fourteen men with traumatic SCI, grouped according to their level of injury. INTERVENTIONS: Subjects performed 2 maximal medicare power wheelchairs exercise tests 1 week apart, in random order and under a counter-balanced design. One test was done with and the other without graduated elastic stockings (21 mmHg). MAIN OUTCOME MEASURES: Blood lactate, blood pressure, heart rate, maximal power output, and oxygen consumption (Vo2). RESULTS: Postexercise venous lactate concentration was reduced in SCI subjects with lesion levels below T6 while wearing graduated elastic stockings during both exercise and recovery (10.9+/-3.9 mmol/L vs 12.5+/-4.6 mmol/L, P<.05). There were no significant differences in submaximal and maximal values (heart rate, Vo2, power output) between subjects tested with and without graduated elastic stockings. CONCLUSIONS: Wearing elastic stockings affects postexercise responses by decreasing lactate concentration in well-trained, low-level paraplegic patients after a maximal exercise. The relatively low pressure generated by the stockings may not, however, influence the venous system enough to produce improved performance and cardiovascular responses

The purpose of this study was to determine the effect of crank length and cadence on mechanical efficiency in hand cycling. The RPE data supported this finding, with a tendency for lower ratings with the 180 mm crank (9 +/- 2 vs. 10 +/- 3). The short crank length when used at 85 rev min(-1) was found to be the most efficient (GE 21.4 +/- 3.1%). In conclusion, crank length has a significant effect on ME in hand cycling. A shorter crank length of 180 mm was found to be more efficient than the 220 mm, regardless of pedal rate during hand cycling

Demographic and socioeconomic factors associated with disparity in wheelchair customizability among people with traumatic spinal cord injury
OBJECTIVES: To determine if a standard of care for wheelchair provision exists within the participating centers and if there is disparity in wheelchair customizability among the study sample. DESIGN: Convenience sample survey. SETTING: Thirteen Model Spinal Cord Injury Systems that provide comprehensive rehabilitation for people with traumatic spinal cord injury (SCI) and that are part of the national database funded through the US Department of Education. PARTICIPANTS: A total of 412 people with SCI who use wheelchairs over 40 hours a week. INTERVENTION: Survey information was obtained from subjects via telephone and in-person interviews and from the national database. Collected information included age, race, education, level of injury, and wheelchair funding source. MAIN OUTCOME MEASURES: Number and type (manual or power) of wheelchairs. Wheelchair customizability as defined by design features (eg, adjustable axle position, programmable controls). RESULTS: Ninety-seven percent of manual wheelchair users and 54% of power wheelchairs users had customizable wheelchairs. No power wheelchair user received a wheelchair without programmable controls. Minorities with low socioeconomic backgrounds (low income, Medicaid/Medicare recipients, less educated) were more likely to have standard manual and standard programmable power wheelchairs. Older subjects were also more likely to have standard programmable power wheelchairs and www.easymedonline.com. Effects of training programs for spinal cord injury
INTRODUCTION: www.easymedonline.com exercise training programs in patients with spinal cord injury (SCI) were largely studied to determine different types of adaptations. The aim of specific rehabilitation is to obtain maximal gains in quality-of-life (QoL) after SCI. OBJECTIVE: To review the literature on the efficiency of training programs for SCI. METHODS: We searched the MEDline database with the keywords SCI, paraplegia and quadriplegia and synonyms, then combined them with one of the following terms: rehabilitation, training, exercise conditioning, physical fitness, exercise prescription, adaptation, effect, or benefit. We found 65 articles related to the physiological and psychological effects of training programmes on patients with SCI. RESULTS AND DISCUSSION: Training programs after SCI offer reconditioning cardiorespiratory, cardiovascular, cardiac, metabolic, bone, biomechanical, muscle adaptation, and QoL benefits. Reconditioning training increases VO2 max, reverses leg vascular resistance in the paralyzed legs and has possible cardiac and neural adaptations, favorable catecholamine responses and effects on platelet aggregation. Reconditioning can also modify lipid profile, reduce risk for cardiovascular diseases, prevent osteoporosis and increase maximal upper-extremity muscle strength, sprint power wheelchairs
output and maximal power output. This effect allows for considerable improvement in mechanical efficiency and wheelchair propulsion technique. CONCLUSIONS: Reconditioning training programs after SCI have a direct impact on function and QoL, permitting participation in physical activities in addition to daily living activities in subjects with SCI.

Doors are a big issue. People in power wheelchairs, scooters etc., need to place themselves to the side of the door to open it. Objects such as trash cans, tables and chairs placed next to the door result in a situation where a disabled person cannot open the door. Please make sure that a 3-foot-wide space is available between the side of the door that opens and any object placed along the wall.
Another issue is bathrooms. If there are multiple stalls available please do not choose the wheelchair accessible stall if you do not need it. It is frustrating and potentially unpleasant to have to wait for the bathroom stall because it is occupied when three other stalls are available.
Wheelchairs, scooters and crutches require more maneuvering room than just the typical human body. Objects piled in hallways can create impassible situations. Imagine not being able to go to a meeting or your office because the hallway was blocked and there was no way around.
Even though it seems like sitting in a wheelchair or scooter requires no effort, disabled people are often plagued by fatigue. And, little tasks such as opening doors can require a great deal of effort. Think about how we would feel to go about the day with a bungee cord around our legs and arms.

New design and development of a manual wheelchair for India
PURPOSE: The most common methods of delivering assistive technology in developing countries are charitable donation and workshops. This describes a new approach to solving the problem, a collaboration undertaken by a US-based lab and a manufacturer in India to produce quality wheelchairs and handicap scooters . One goal is to publicize the design free of charge to manufacturers and interested parties world-wide. The process, a demonstration of a new technology transfer method, and the product, an adult manual wheelchair, are described. METHOD: An iterative process occurred over four years to design and produce the medical scooters . This consisted of prototypes, small production runs, ANSI/RESNA testing, hardness and tensile testing and informal user testing. RESULTS: The design is a manual folding cross-brace design with several points of adjustability. Final pre-production prototypes experienced fastener failures during durability testing. Higher grade bolts were specified. Trial-run production has begun. An ANSI/RESNA wheelchair test lab was constructed in India. Subsequent projects include power and pediatric tilt-in-space wheelchairs. CONCLUSIONS: The approach seems promising as a method to improve the quality of assistive technology available in India and begin to meet the vast need in India. Pitfalls encountered throughout the collaboration are described in this paper along with solutions to remedy these problems for future projects

INTERVENTIONS: Participants were randomly assigned to receive 10 treatments of either acupuncture or www.easymedonline.com, invasive sham acupuncture (light needling of nonacupuncture points). MAIN OUTCOME MEASURE: Changes in shoulder pain intensity were measured using the power wheelchairs User’s Shoulder Pain Index. RESULTS: Shoulder pain decreased significantly over time in both the acupuncture and the sham acupuncture groups (P=.005), with decreases of 66% and 43%, respectively. There was no significant difference between the 2 groups (P=.364). There was, however, a medium effect size associated with the acupuncture treatment. CONCLUSIONS: There appears to be an analgesic effect or a powerful placebo effect associated with both acupuncture and sham acupuncture. There was a medium treatment effect associated with the acupuncture, which suggests that it may be superior to sham acupuncture. This observation, along with the limited power, indicates that a larger, more definitive randomized controlled trial using a similar design is warranted
